Enrollment Information
Proof of legal guardianship, birth certificate/proof of age, medical shot record (HRS form 680), two proofs of residency showing guardian’s name and street address (utility bill, voter registration card, property tax bill, rental/lease documents), transfer papers from previous school with school’s address and phone number.
Students entering kindergarten must be five years old on or before September 1, 2008. Please contact individual schools for documents required.
