How to Register at Griffin

Please all the office at 853-6020 to ask any questions before you come to the school.

Documentation required to bring to school:

1. Child's Birth Certificate

2. Physical Exam within 12 months of registration

3. Florida Certificate of Immunization (blue card)

4. Two proofs of residency: 

• apartment rent receipt or home lease agreement, mortgage, property tax record

• one current utility bill (electric, gas, phone, cable, water)

• current voter registration document

• proof of current government benefits

5. Withdrawal and transfer form from the school student has been attending.

    It's a good idea to bring your child's current IEP or 504 Plan if your child is being served in either of these areas. Be prepared to fill out several forms that include information like emergency contacts, doctors' phone numbers, insurance plan ID numbers, and medical authorization forms if your child takes medication during the school day. Remember to include any allergy information on the medical card.
