Union
Academy
Clinic Information
Union Academy is
VERY fortunate to have a full-time clinic aid available to assist students and
staff. In utilizing the clinic, certain procedures must be followed.
|
Sick or Ill Students |
| Sick or ill students are usually
sent to the clinic by a teacher. A student should report to class to receive a pass to the clinic
when feeling ill.
* Students should not go to the clinic
on their own without checking in with their teacher, unless their illness
requires them to. |
|
Medication Distribution |
| Students that take
medication during school hours must have an Authorization for Medication*
form, signed by their physician and parent, on file
in the clinic.
* This form is also required for OVER
THE COUNTER medications such as Tylenol, aspirin, cold medication, etc. |
|
Parent Contact Information |
| It is EXTREMELY important for your
emergency contact information to be up-to-date and accurate. It
is very difficult to reach parents when the information provided is incorrect.
* Note:
Anyone coming to pick up a student MUST be listed on the student's
emergency card indicating they have permission to pick the student up. |
