Clinic Information


Union Academy is VERY fortunate to have a full-time clinic aid available to assist students and staff.  In utilizing the clinic, certain procedures must be followed.

Sick or Ill Students

Sick or ill students are usually sent to the clinic by a teacher.  A student should report to class to receive a pass to the clinic when feeling ill.

* Students should not go to the clinic on their own without checking in with their teacher, unless their illness requires them to.

 

Medication Distribution

Students that take medication during school hours must have an Authorization for Medication* form, signed by their physician and parent, on file in the clinic.

* This form is also required for OVER THE COUNTER medications such as Tylenol, aspirin, cold medication, etc.

 

Parent Contact Information

It is EXTREMELY important for your emergency contact information to be up-to-date and accurate.  It is very difficult to reach parents when the information provided is incorrect.

* Note:  Anyone coming to pick up a student MUST be listed on the student's emergency card indicating they have permission to pick the student up.

 

 Shot Requirements for
7th Grade

* 1st and 2nd MMRs (one shot each)
* Polio (4 doses)
* DPT (5 shots)
* Td Booster (1 shot within 5 years of entering 7th grade)
* Hepatitis B Series (3 shots - or at least the series started)
