New Student Registration Procedures

Registration Requirements for
Out of State, County, or
New to Public School

1. Two proofs of residence (any two):

   1. property tax records, mortgage documents or deeds, apartment or home lease, or copy of rent or mortgage payment

   2. any one (1) utility bills such as electric, gas, phone, cable, or water

   3. voter registration card

   4. documentation of participation in a government assistance program

2. Proof of legal guardianship

3. Copy of birth certificate (if foreign, must have English translation)

4. Copy of immunization records (must be on blue Florida form)

5. Physical exam (current within the last 12 months)

6. Copy of withdrawal and transcript from previous school (if foreign, must have English translation)

Parents are responsible for obtaining the records that are needed for registration.

Also be advised that if the parent resides in Polk County, it is the residence of the parent that determines the school assignment unless either HRS or the Juvenile Court has given custody to another adult out of concern for the risk to the child.
