REGISTRATION:

 

Requirements for students entering Polk County Schools from OUT OF STATE:

  1. Birth Certificate (must be a certified copy from the state, not a hospital certificate
  2. Physical (must be within one year)
  3. Certificate of Immunization/Polk County (shots must be recorded on blue Florida Health card)
  4. 2 Proofs of Residence (the proofs must be from different categories and show name and physical address)
    Category 1: Apartment or home lease agreement, mortgage document, or property tax record.
    Category 2 : Current utility bill (electric, gas, phone, cable, water)
    Category 3:  Voter registration document
    Category 4:  Proof of government benefits (disability, Medicare, food stamps, HRS correspondence)
  5. Student Social Security Number
  6. Complete address of former school

IN-COUNTY/STATE

  1. Withdrawal from former school (do not need in the summertime)
  2. 2 proofs of residency

PRIVATE SCHOOLS

  1. 2 proofs of residency
  2. Letter regarding placement

ESE STUDENTS

  1. Parental permission for release of ESE records.

SCHOOL HOURS:

Classes begin at 8:00 a.m. and end at 2:20 p.m.

Campus supervision begins at 7:30 a.m.  The school is not responsible for students who are dropped off before 7:30 a.m. 

Students are to be picked up no later than 2:50 p.m.


CAR RIDERS:

In the morning, please pull all the way up.  It allows us to unload 15 cars at a time.

Pick up times in the afternoon are as follows:

2:20 - car tag #'s 1-149
2:25 - car tag #'s 150-299
2:30 - car tag #'s 300-400

If your child is going to be a car rider at any time during the school year, please check with the office about getting a car tag number.

Parents, please do not park and walk up.  It is not safe for the parents or the students.


BUS RIDERS:

The front office can provide you with a bus schedule.


CLASS SCHEDULES:

Each classroom has a different schedule which includes at least 90 minutes of reading
instruction, 60 minutes of math instruction, 60 minutes of writing instruction,
social studies and science instruction, plus 20 minutes of lunch, 15 minutes of recess,
and 45-50 minutes of special area instruction (P.E., music, art, computer lab, library).
Classroom teachers will provide parents with the schedule for their child's class.
